Earlier this week, I was talking with colleagues about what we mean by organisational health. Within Tricordant, we are clear on what we mean, but we are not sure everyone else has the same level of understanding.

Multi-Disciplinary Team (MDT) working is increasingly recognised as a key enabler of integrated, patient-centred care in the NHS.

Tricordant worked with the Partners and leadership team of Brookside General Practice, which has a list size of 31,000 patients, to support their leadership transition to a new Partnership portfolio model and a new service model based around multi-disciplinary Patient Health Teams.
